What Is the PTCB Exam?

The PTCE (Pharmacy Technician Certification Exam) is the national certification exam administered by the Pharmacy Technician Certification Board (PTCB) through Pearson VUE testing centers. Passing it earns you the CPhT (Certified Pharmacy Technician) credential — required by law in some states (including Texas) and strongly preferred by most employers everywhere else. An updated version of the PTCE launched January 6, 2026, with a revised four-domain content outline (used throughout this guide and CertPrepAcademy's practice questions). Exam facts: • 90 questions total — 80 scored, 10 unscored pretest questions mixed in throughout (you won't be told which ones, so treat every question as if it counts) • 110 minutes to complete • Passing: scaled score of 1,400 or higher, out of a 1,000–1,600 range • Format: multiple-choice, one correct answer per question • Fee: $129 total ($50 application + $79 exam); resets to $129 on each retake • In-person only, at more than 1,400 Pearson VUE test centers nationwide Eligibility: a high school diploma or equivalent, no disqualifying criminal or pharmacy-board history, plus one of two pathways — completion of a PTCB-recognized education/training program, or at least 500 hours of verified pharmacy technician work experience.

What's on the PTCE — The Four Domains

The 2026 PTCE is divided into four knowledge domains. Study time should track these percentages — Medications is the largest domain by a wide margin. Medications (35% of the exam) • Generic names, brand names, and drug classifications • Therapeutic duplications across a patient's medication profile • Drug interactions and contraindications — drug-drug, drug-supplement, drug-lab, drug-nutrient, and drug-disease • Strengths, dosage forms, and routes of administration • Medication side effects, adverse effects, and allergies • Indications — what conditions and symptoms a medication treats • Drug stability — oral suspensions, insulins, reconstitutables, injectables, vaccines • Proper storage of medications — temperature ranges, light sensitivity, restricted access Patient Safety and Quality Assurance (23.75% of the exam) • High-alert medications and look-alike/sound-alike (LASA) drug pairs • Error prevention strategies — Tall Man lettering, separating similar-looking inventory, leading/trailing zero rules, barcode scanning, limiting error-prone abbreviations • Issues requiring pharmacist intervention — drug utilization review (DUR), adverse drug events, OTC recommendations, therapeutic substitution, misuse, adherence • Event reporting procedures — medication errors, MedWatch, VAERS, near-miss reporting, root-cause analysis, continuous quality improvement • Types of prescription errors — wrong dose, quantity, patient, drug, or route • Infection prevention and cleaning standards — hand hygiene, PPE, cleaning counting trays and equipment Order Entry and Processing (22.5% of the exam) • Pharmacy calculations and Sig codes — ratios, proportions, conversions, b.i.d./t.i.d.-style abbreviations, days-supply and dose math • Equipment and supplies for drug administration — diabetic supplies, inhaler spacers, syringes, filters, dilution solutions, nebulizers • Lot numbers, expiration dates, and National Drug Code (NDC) numbers • Identifying and returning medications and supplies — dispensable vs. non-dispensable, credit return, return-to-stock, reverse distribution Federal Requirements (18.75% of the exam) • Storage, handling, and disposal of controlled and hazardous substances • Controlled substance prescriptions (new, refill, transfer) and DEA schedules • Receiving, ordering, and destroying controlled substances • Federal restricted drug programs — pseudoephedrine sales limits, REMS (Risk Evaluation and Mitigation Strategies) • FDA medication recall procedures • Product serialization and tracking under the Drug Supply Chain Security Act (DSCSA) Medications is the largest domain by far, but Federal Requirements is where candidates most often lose points they didn't expect to — the rules are specific and easy to mix up under time pressure.

How Long to Study — A Realistic 4-Week Plan

Most candidates who pass on their first attempt study consistently for 4–6 weeks, whether coming out of a training program or preparing through the work-experience pathway. Week 1 — Medications, Part 1. Generic/brand names and classifications, therapeutic duplications, drug interactions and contraindications. These build the foundation everything else sits on top of. Week 2 — Medications, Part 2. Dosage forms and routes of administration, side effects and allergies, indications, drug stability, and storage requirements. By the end of week 2 you've covered the domain worth 35% of the exam. Week 3 — Patient Safety and Quality Assurance, then Order Entry and Processing. High-alert and LASA drugs, error prevention strategies, pharmacist-intervention triggers, and event reporting. Then pharmacy calculations (practice the Sig-code and dosage math until it's automatic), equipment/supplies, NDC numbers, and return procedures. Week 4 — Federal Requirements, then full practice tests. Controlled substance storage, prescriptions, and DEA schedules; restricted drug programs; recalls; DSCSA serialization. Finish the week with at least one full 90-question timed practice test, reviewing every explanation — not just the ones you missed. Cramming Federal Requirements the night before is a common mistake — the rules are specific enough that they need real repetition, not a single read-through.

Why Candidates Fail the PTCE (and How to Avoid It)

1. Underestimating Federal Requirements. At 18.75% of the exam, it's the smallest domain — but the rules (DEA schedules, controlled substance disposal, pseudoephedrine limits, DSCSA serialization) are precise and easy to confuse under pressure. A little repetition here goes a long way. 2. Skipping the calculation practice. Pharmacy calculations and Sig-code questions appear throughout Order Entry and Processing. The math itself isn't hard, but test-day anxiety causes careless errors. Practice conversions and dose calculations until they're automatic. 3. Treating Patient Safety as common sense. Error-prevention rules like Tall Man lettering and leading/trailing zero conventions have specific, testable right answers — they're not just general safety awareness. 4. Confusing technician scope with pharmacist scope. PTCE questions are written for what a pharmacy technician does, not what a pharmacist does. When a question asks what the technician should do, the correct answer is almost never to counsel a patient on therapy or make a clinical judgment call — it's to prepare, document, verify, or refer to the pharmacist. 5. Running out of time. 110 minutes for 90 questions is a little over a minute per question. Flag anything you're unsure about and come back to it rather than losing several minutes on one question.

How to Use PTCB Practice Tests Effectively

Start with a diagnostic test before you begin studying, to see which of the four domains you already know well and which need the most work. During weeks 1–3, use domain-specific topic quizzes right after studying each domain — testing yourself immediately after learning a topic reinforces it better than waiting until the end. In week 4, switch to full 90-question timed tests to simulate real exam conditions and pacing. What is the PTCB exam pass rate?

PTCB does not publish an official first-attempt pass rate as directly as some other certifying bodies. Third-party reporting on recent years suggests a first-attempt pass rate somewhere in the 60–70% range, though estimates vary by source and year. Candidates coming from a PTCB-recognized training program and those who complete structured practice testing before their exam date consistently report better outcomes than those who study passively or cram in the final week.

How many questions are on the PTCB exam?

The PTCE has 90 multiple-choice questions. Of those, 80 are scored and 10 are unscored pretest questions PTCB uses to develop future exam forms. You won't be told which questions are pretest, so answer every question as if it counts. You have 110 minutes to complete the exam — a little over a minute per question.

What is a passing score on the PTCB exam?

The PTCE uses scaled scoring on a 1,000–1,600 range, and you need a scaled score of 1,400 or higher to pass. Scaled scoring adjusts for small differences in difficulty between exam forms, so a 1,400 on one version is equivalent to a 1,400 on any other version. PTCB does not publish the exact raw number of correct answers that corresponds to 1,400, which is why consistent strong performance on full-length practice tests — not a single lucky run — is the more reliable signal that you're ready.

How often does the CPhT credential need to be renewed?

The PTCB CPhT certification must be renewed every two years. Renewal requires 20 hours of continuing education (CE) during that period, including at least 1 hour specific to pharmacy law and 1 hour specific to patient safety. You can start the renewal process up to 60 days before your certification expires, and the application deadline is 30 days before expiration. Some states, including Texas, also layer their own state-specific CE requirements on top of PTCB's renewal requirement.
