Therapeutic Duplications is worth 4.4% of the PTCE. It tests whether you can scan a patient's medication profile and spot two or more drugs from the same class — or with the same active ingredient — that together provide no added benefit and raise the risk of additive toxicity. The core skill is recognizing that two differently-named drugs can belong to the same class, and that hidden ingredients in combination products create silent duplications.
Same-Class Duplications
A duplication exists when two drugs share a pharmacologic class even if their names look unrelated. Classic examples: ibuprofen + naproxen (two NSAIDs → additive GI bleeding and renal risk); sertraline + fluoxetine (two SSRIs → serotonin syndrome risk); alprazolam + lorazepam (two benzodiazepines → additive CNS/respiratory depression); omeprazole + pantoprazole (two PPIs); lisinopril + losartan (ACE inhibitor + ARB → hyperkalemia, kidney injury, no added benefit). Hidden-Ingredient Duplications (the Acetaminophen Trap)
The most dangerous duplications are hidden inside combination products. A patient taking Norco (hydrocodone + acetaminophen) or Percocet (oxycodone + acetaminophen) who also takes OTC Tylenol can easily exceed the acetaminophen ceiling — traditionally 4,000 mg (4 g) per day for adults, with some OTC labels now recommending no more than 3,000 mg/day — risking hepatotoxicity, the leading cause of acute liver failure in the U.S. The same trap applies to NSAIDs hidden in cold/sinus combos and to diphenhydramine hidden in OTC "PM" sleep aids alongside a separate Benadryl dose.
Always total the acetaminophen from every source — prescription opioid combos plus OTC Tylenol is the classic overdose duplication.
Duplications vs. Legitimate Combination Therapy
Not every same-category pairing is an error. Prescribers intentionally combine agents with different mechanisms — for example a long-acting basal insulin (glargine) with a rapid-acting mealtime insulin (lispro), or an inhaled corticosteroid with a long-acting beta agonist for asthma. These are complementary, not duplicative. A true duplication is two drugs doing the same job by the same mechanism. When unsure, the technician flags the profile for the pharmacist rather than assuming — verifying duplications is ultimately a pharmacist judgment call.
Where Duplications Come From
Duplications commonly arise from multiple prescribers (a specialist and a PCP each prescribe a statin), from brand-to-generic confusion (a patient continues brand Prilosec while a new generic omeprazole is dispensed), from formulary switches, and from OTC self-treatment layered on top of prescription therapy. A complete, reconciled medication profile — including OTCs and supplements — is the only reliable defense, which is why techs collect a full medication history at intake.
Brand-plus-its-own-generic (Prilosec + omeprazole) is a duplication patients rarely recognize on their own.
